When you start a campaign, you need a form you can use for the campaign. This can be one of the templates that are already available in your form list, or you can create a custom form. This way, you can exactly ask what you want, in the way you want.
Do you want to create a form? Choose forms in your Ratecard dashboard.

And click on the + button:

Step 1: How to setup your custom form
Fill in the form name and language
Name the form after the function of the form you're about to create. You can also select the language you want the form to be in and what you would like to measure. In the advanced settings, you can find the branding of the form.

Step 2: How to build your form
After filling in everything in the setup you can select one of our templates or create your own template.
In the next step you can build your form.

Edit, copy & remove blocks
Each building block can be adjusted, copied or removed. You can do this by clicking Select action and pick the action you'd like.


If you choose to edit the element, you are able to create your own texts for every item.

Adjusting the order of the building blocks
By dragging the blocks from the left side to the right blocks, you can easily add elements (multiple choice questions/dropdowns/relation ships/etc) you want to be in the form. You can also adjust the order of all building blocks by dragging them to another position.

You can add a custom question, extra information, you can choose the scale and presentation.

The presentation means the appearance of the rating icons on the form:


Next to the common rating question, you also have two options to ask for NPS. Read here what NPS exactly means and how you can use this correctly in a form.
Check the preview
While creating your custom form, it's possible to see if the form looks the way you want to and contains all items you want to request. Do this by clicking on the Preview button at the top of the page.

Step 3: Publish your form
If you're satisfied with everything, click Publish to publish your form.

The form appears now in your list with all forms and can be used in your campaigns. Create your form now!
